Pocahontas Area Staff & Students:
We are excited to be working with the Pocahontas School again this year to give the seasonal influenza vaccine. This vaccine will protect against all three influenza strains that are expected to circulate this year. We will hold the vaccination clinic at PAC on OCTOBER 16, 2025.
The vaccine consent form includes options allowing you to either accept or refuse the vaccination for student.
Read the “what you need to know” form included with this letter about the disease and the vaccine.
Sign and date the included consent form to accept vaccination.
Return the consent form to the school no later than Tuesday, October 14, 2025, so we can have an estimated count of how many vaccines will be needed. Late forms will be accepted the day of if you should change your mind.
Please include a copy of updated insurance card or be very specific which type of insurance your child has (Hawki, Amerigroup, Blue Cross Blue Shield, etc.)

Unity Point Clinic and ask to speak with Shelby. We will only be giving the flu vaccine; no other vaccines will be given on this day. I can be reached at 712-335-5632 from 9 AM to 4PM, Monday through Friday.
